Dry scalp or dandruff? How to tell the difference
Flakes on your shoulders could be dandruff, a dry scalp or something else. They need different care, and some simple clues help tell them apart. Here is how to read the signs and when to get help.
White flakes on a dark shirt are one of the most common scalp complaints. Most people call every flake "dandruff", but a dry scalp and dandruff are not quite the same thing, and the care that helps one may not help the other. Flaking can also be a sign of other skin conditions. This guide explains the differences, what usually helps, and when to get your scalp checked.
What is dandruff?
The NHS describes dandruff as a common skin condition that is not harmful and cannot be caught from someone else. It causes white flakes of skin on the scalp and in the hair, which are more noticeable in dark hair and when they fall onto your shoulders. The scalp may also feel dry and itchy 1.
Dandruff is not caused by poor hygiene, although it may be more obvious if you do not wash your hair regularly. It can be linked to a type of yeast that lives naturally on the skin, and to skin conditions such as eczema. Stress and cold weather may make it worse 1. The AAD adds that dandruff can be linked to oily skin and to hair care habits 2.
What is a dry scalp?
Dry skin happens when the outer layer of the skin lacks moisture, which leads to tiny cracks in the surface 3. Dry skin looks dull and feels rough and scaly. It is less supple, and when the dryness is severe it can become inflamed 3. The scalp can be affected like any other skin.
DermNet lists common causes of dry skin 3:
- Low humidity and excessive air conditioning
- Direct heat from a fan or heater
- Excessive bathing
- Soaps, detergents and solvents
- Products containing alcohol
- Friction from rough fabrics or scrubs
Dry skin also becomes more common with age and is seen in some medical conditions, such as an underactive thyroid and kidney disease, and with certain medicines, including some diuretics and oral retinoids 3.
Telling them apart
There is no single test you can do at home, and the two can overlap. These clues can help:
- Scalp feel: leans towards dandruff: Can feel oily or normal; leans towards dry scalp: Feels tight and dry
- Linked factors: leans towards dandruff: Oily skin, yeast, stress; leans towards dry scalp: Air conditioning, hot water, harsh soaps
- Other dry skin: leans towards dandruff: Not usually; leans towards dry scalp: Often dry skin elsewhere too
- Response: leans towards dandruff: Often improves with anti-dandruff shampoo; leans towards dry scalp: Often improves with gentler washing
These clues are a guide, not a diagnosis. If you are unsure, the NHS advice is simple: follow the self-care steps and see a doctor if things do not get better within a month 1.
Other conditions that cause flakes
The NHS lists several conditions that can look like dandruff 1:
- Seborrhoeic dermatitis: scaly, itchy, red patches on the scalp, face and other areas. See seborrhoeic dermatitis.
- Scalp ringworm (tinea capitis): a red or silvery rash, sometimes with patchy hair loss. See tinea capitis.
- Eczema: dry, red, flaky and very itchy skin.
- Contact dermatitis: red, irritated skin, sometimes with blisters, after products such as hair dye, sprays or gels.
- Psoriasis: red, crusty, sore patches covered with silvery scales. See scalp psoriasis.
What helps dandruff
The NHS suggests an anti-dandruff shampoo containing one or more of ketoconazole, coal tar or salicylic acid, used for up to 4 weeks to see if it helps. You may need to try more than one type 1. The AAD also lists zinc pyrithione, sulfur and selenium sulfide, and suggests alternating shampoos with different active ingredients if one does not work. Follow the label, and lather the shampoo onto your scalp 2.
The AAD notes that people with oily scalps or straight hair often do better shampooing more often, while people with coarse or curly hair may use a dandruff shampoo about once a week 2. See dandruff and hair loss.
What helps a dry scalp
DermNet's advice for dry skin in general can be adapted for the scalp 3:
- Use lukewarm water, not hot water.
- Take short showers instead of long hot soaks.
- Replace harsh soap with a gentle, soap-free cleanser.
- Drink enough water.
- Remove aggravating factors, such as harsh products or scrubbing.
Moisturisers are the main treatment for dry skin 3. For the scalp, that may mean a light, fragrance-free leave-in product on the skin or a gentle conditioner. The AAD recommends conditioner after every wash to protect the hair itself 4.
If your scalp is dry, washing a little less often can help. Many people in Malaysia sit in air-conditioned rooms all day, which can also dry the skin 3. See how often to wash hair.
Does flaking cause hair loss?
Dandruff itself is not harmful 1. However, conditions that cause flaking, such as scalp ringworm, can cause patchy hair loss 1. Heavy scratching can also damage hair. If flaking comes with bald patches, broken hairs or pain, get it checked.
When to see a doctor
The NHS advises seeing a doctor if 1:
- You still have dandruff after using an anti-dandruff shampoo for at least 4 weeks
- Your dandruff is bad or your scalp is very itchy
- Your scalp is red or swollen
- You have flaky, itchy patches on your face or other parts of your body
The doctor can check for skin conditions that may be causing the flakes 1. If a dry, itchy scalp turns into a rash, DermNet notes that dry skin can lead to a form of eczema, which may need a steroid cream 3.
The bottom line
Dandruff and dry scalp both cause flakes, but they have different triggers. Dandruff often responds to an anti-dandruff shampoo. A dry scalp usually needs gentler washing, cooler water and fewer harsh products. If flakes last longer than a month of self-care, or come with redness, swelling or hair loss, see a doctor.
Frequently asked questions
Is dandruff the same as a dry scalp?
Not exactly. Dandruff is a common skin condition linked to yeast and oily skin 1 2. Dry scalp comes from a lack of moisture in the skin 3.
Is dandruff caused by not washing?
No. The NHS says dandruff is not caused by poor hygiene, although it may be more visible if you wash less often 1.
How long should I try an anti-dandruff shampoo?
The NHS suggests up to 4 weeks, and seeing a doctor if it has not improved 1.
Can air conditioning dry out my scalp?
Excessive air conditioning and low humidity are listed causes of dry skin 3.
