SMP aftercare: the first weeks and the long term

Good aftercare helps scalp micropigmentation (SMP) heal well and may help the pigment last. Because SMP is a form of tattoo, general tattoo-care advice applies, with a few scalp-specific points for Malaysia's heat and humidity.

Scalp micropigmentation (SMP) places pigment in the skin using tattoo-style equipment 1. That means each session leaves small wounds that need time to heal. Your practitioner should give you written aftercare instructions, and these should come first. This guide explains the general principles, based on tattoo-care advice from dermatology and public health sources, so you understand why the instructions matter.

The first few days

  • Keep the scalp dry for the period your practitioner advises. Soaking a new tattoo can slow healing 2
  • Do not scratch or pick at flakes or small scabs. Picking can slow healing and may lead to infection 2
  • Wash your hands before touching the treated area, and do not let others touch it until healed 2
  • Avoid heavy sweating, such as gym sessions, sports or saunas, until your practitioner says it is fine
  • Sleep on a clean pillowcase and change it often
  • Some redness, tenderness or mild flaking can be expected at first

Washing and moisturising

  • When you are allowed to wash, use lukewarm water and a gentle, fragrance-free cleanser, and pat dry rather than rubbing
  • Only use creams your practitioner recommends, and do not overuse them 2
  • Dermatologists advise that petroleum-based products such as petroleum jelly can cause tattoo ink to fade, and suggest a water-based lotion if the skin feels dry once healed 3
  • Avoid scrubs, exfoliating shampoos and strong hair products on the area until it has healed

Sun, heat and humidity in Malaysia

Malaysia's strong sun and humidity make aftercare especially important.

  • Cover up while healing: a loose, clean cap or hat may be advised, but ask your practitioner first, as tight headwear can rub the area
  • Ultraviolet light can fade some tattoo inks. Once healed, dermatologists suggest protecting tattooed skin with a broad-spectrum, water-resistant sunscreen of SPF 30 or more, reapplied at least every two hours outdoors 3
  • Do not apply sunscreen before the area has healed, unless your practitioner says otherwise 2
  • Avoid tanning beds and sunlamps 3
  • See hair and scalp care in heat and sun for more on protecting your scalp

Swimming and water

  • Avoid swimming in pools, the sea or rivers until the area has healed 2
  • Avoid long hot showers, steam rooms and hot tubs during healing

Shaving and hair care

  • If you keep a shaved look, ask when you can shave again after each session. Shaving too soon can irritate healing skin
  • Once healed, regular shaving or very short clipping keeps the SMP illusion consistent
  • Use a clean, sharp razor or clipper, and avoid sharing them

Tudung, caps and helmets

  • If you wear a tudung, ask your practitioner when to resume and choose a clean, loose, breathable fabric at first
  • Motorcycle helmets can rub and trap sweat. If you ride, ask about timing and consider a clean, breathable liner

Between sessions

SMP is usually built up over several sessions. In one published series, sessions were done a week apart with a touch-up a month later 4. Follow aftercare after each session, not just the first. Keep your appointments so your practitioner can check healing and adjust density.

When to seek help

Contact your practitioner, or see a doctor, if you notice:

  • Redness that spreads, or swelling that gets worse
  • Increasing pain, warmth or pus
  • Fever or feeling unwell
  • An itchy rash or bumps on the treated area, especially if it appears later

Dermatologists note that skin reactions to tattoo ink can happen straight after a tattoo or even years later 5. In one SMP series, one of 22 patients had an allergic reaction 4. Do not try to treat a reaction with home remedies.

Long-term care

  • Protect your scalp from the sun to help the pigment look its best (how long SMP lasts)
  • Keep your scalp healthy, and get scaly or itchy patches checked
  • Expect touch-ups over time

The bottom line

Follow your practitioner's instructions first. In general: keep the area clean and dry at first, do not pick, avoid sweating, swimming and sun while healing, and use sunscreen once healed. Seek help early for signs of infection or reaction, and keep your follow-up appointments so healing can be checked. See SMP in Malaysia and SMP regrets and removal.
